Departmental Affiliation Form

Purpose

This document is completed by Biomedical Ph.D. and admitting umbrella Ph.D. programs after a student's first-year of Ph.D. study to affiliate the student with a lab or degree-granting program. This form is used to establish a student’s initial departmental or program affiliation. It is not a change‑of‑advisor form and should not be used to update or change faculty advisors.
