Health Requirements
Proof of Immunization:
North Carolina law requires all students enrolled in colleges or universities in this state to present proof of certain required immunizations on or before the first day of matriculation. To access the immunization form, you will need your Duke NetID and Password. This information will be mailed to you by surface mail after you have been admitted to the Graduate School and accepted the offer of admission. Please contact the Student Health Center for more information on immunization requirements.
Health Insurance:
Information concerning health insurance including details about plan coverage, contact information, claims submission and dependent coverage is available from the Student Health Center.
